Optimal Performance is based on many things.

Although we specialize in the "mental" aspects of the game, we want to help you stay aware of, and help you plan to optimize, all the factors that contribute to your performance.

By answering the following questions, we will be able to provide you with a visual picture of your performance potential and we will be better prepared to help you plan for your optimal performance.

Instructions

Before your first appointment:

A five point scale will be used to help you assess each question.

1 = Optimum, this is a most positive factor contributing to your peak performance
2 = Positive, although not optimum or ideal, this is still a positive factor
3 = Neutral, this neither helps nor sets you back
4 = Minus, this factor holds you back or moves you backwards to some degree,
it is a somewhat negative factor
5 = Liability, definitely a liability, this factor is a major barrier to your peak performance
